The East Longmeadow campus of care offers a range of housing and healthcare options to meet a variety of needs associated with aging. East Longmeadow Memory Care Assisted Living provides all-inclusive memory care assisted living, with skilled nursing and long-term care offered next door at East Longmeadow Skilled Nursing Center.

Our beautifully appointed, compassionate Life Enrichment Memory Care Program offers resident-focused care for those living with Alzheimer’s and other forms of memory loss and dementia, helping residents experience the highest possible quality of life.

When scheduling surgery or following a health-related event, choose East Longmeadow Skilled Nursing Center for help getting your loved one back to their routine. Our skilled nursing center also offers a higher level of memory care if needs progress beyond those available in assisted living.

Memory care assisted living residents requiring care following surgery or health-related event, or who need a higher level of ongoing care and support, receive priority admission to East Longmeadow Skilled Care Center for short-term rehabilitation or long-term care. Our care providers communicate closely to provide a smooth transition between care settings.

Our Medical Director oversees resident and patient care and establishes program initiatives to promote the health and well-being of our entire campus.
